Ageing can come with changes and challenges. Our Older Adult Community Health Navigator (CHN) can help you manage this stage of your life. The Older Adult CHN supports older adults (59+) and seniors in finding and accessing resources in the community.
The Older Adult CHN helps older adults connect to the supports they need in the community. Areas of support could include:
- providing basic housing information,
- financial forms assistance,
- navigating the health care system,
- navigating dementia resources, and
- connections to other resources in the community as needed.
CHNs specialize in resource finding and navigation for clients with dementia and their caregivers. For more information, read the sections below: Older Adult Resource Navigation and Dementia Resource Navigation.
Our Community Health Navigators (CHNs) are available to assist Older Adults (59+) who live in Stony Plain or Parkland County. They are also able to assist with information and resource navigation to support families and community members of any age experiencing dementia.

The program is open to all residents in the Tri-Municipal Region and is an excellent way for us to work together to help seniors in our community find local information, resources, and support. The Community Health Navigator (CHN) is available to aid with connections in the community for clients living with dementia and their care partners.
Areas of focus include navigating a diagnosis, connections to dementia-related agencies, navigating respite options, education opportunities, and connections, accessing caregiver supports, and connections to community programs.
Every person’s journey with dementia is different and a diagnosis of dementia can include different challenges. CHNs are here to help make the process of getting connected to supports easier for you and your loved ones.
How to Refer
