Older Adult Community Health Navigator

Ageing can come with changes and challenges. Our Older Adult Community Health Navigator (CHN) can help you manage this stage of your life. The Older Adult CHN supports older adults (59+) and seniors in finding and accessing resources in the community. 

The Older Adult CHN helps older adults connect to the supports they need in the community. Areas of support could include:

- providing basic housing information,

- financial forms assistance,

- navigating the health care system,

- navigating dementia resources, and

- connections to other resources in the community as needed.

CHNs specialize in resource finding and navigation for clients with dementia and their caregivers. For more information click on the area of interest below.
