Community Health Navigator

Community Health Navigators (CHNs) work with WestView PCN targeted patients who have chronic diseases, such as diabetes, kidney disease, heart disease, and lung disease, to improve access to health care and other resources available in their community. 

CHNs work with patients, communities, physicians, and the health care team to provide the best care possible. They are experts in helping patients find health, social, and cultural resources in their communities.

The CHN program can be offered virtually, in person, or a combination of both to support patients living with chronic disease. 

CHNs support patients in various ways that contribute to managing chronic health conditions. Mainly, areas of help fall into one of these categories:

1- System navigation,

2- Patient self-management, and

3- Social connection.

If you are struggling with health challenges such as diabetes, high blood pressure, asthma, or heart disease, a CHN can help you navigate the healthcare system, and connect you to the resources you need.

If you need to see a Doctor or Nurse Practitioner for medical care such as newing a prescription and cannot attend a clinic in person, book an appointment in our Virtual Care Clinic (VCC). This is the new walk-in alternative with flexible appointment times during the day and after hours.
