Practice Facilitator – Full-time, temporary (1-year term)

The Practice Facilitator is a champion for change working collaboratively with family physicians, clinic managers, and Primary Care Network (PCN) staff. Must exhibit readiness in quality improvement to plan, implement, and evaluate clinical initiatives that target patient-centered care within the Patient’s Medical Home.

The Practice Facilitator will work as part of the Quality Improvement team to support member clinics through identifying issues and working out solutions aligning with clinic processes and values.

Nurse Practitioner - Indigenous Health (Paul First Nation) - 0.8 FTE

As a Nurse Practitioner (NP) at Paul First Nation, you will play a critical role in providing high-quality, patient-centered care to individuals and families within the Paul First Nation community. You will work collaboratively with a team to deliver a wide range of services, including diagnosing and managing acute and chronic conditions, performing routine physical exams, drawing blood, and promoting health and wellness.

Youth Mental Health Therapist – 0.4 FTE (Contract Based)

We are currently recruiting a Youth Mental Health Therapist (MHT) who will provide comprehensive functional assessment services and care to patients within the Primary Care Network’s Youth Mental Health Clinic (YMHC). The MHT will assess, plan, implement and evaluate behavioural based treatment plans to patients aged 11-25. The MHT will work directly with family physicians in the community, other health professionals, local and regional programming, in a multi-disciplinary environment to ensure a high quality of evidence-based health care to the practice population.

The MHT is expected to be flexible and accommodating to the needs of PCN physicians and other team members within the YMHC, and as approved by the PCN.
