Responsibilities/Duties:

The Specialized Geriatric Services Program (SGS – Clinical Alternative Relationship Plan) in partnership with Alberta Health Services is inviting applications for a Geriatrician / Care of the Elderly Physician to join the Divisions of Geriatric Medicine / Care of the Elderly located in Edmonton, Alberta, Canada. Alberta Health Services is Canada’s largest and only provincial health region providing services for a population of over 3 million people.

This post of 0.2FTE is an unrivalled opportunity for an interested, engaged and enthusiastic physician to be involved in the service of the new care of the elderly service and model of care in AHS Homecare inclusive of the Suburban Rural West Homecare Network (Westview, & Evansburg) and provide cross coverage with other network offices, when needed.

Duties with the Homecare will include assessment and management of frail, medically complex older people in receipt of home care services, home based assessments and targeted interventions for those people either deteriorating in the community or stabilization following an acute or rehabilitative admission. Referrals will be received either from home care, from local primary care providers or from facility-based physicians or transition care coordinators. This role would be supported by the multi professional team within homecare. This role will have the opportunity to facilitate engagement of primary care and consider innovative collaborations with PCNs.

The SGS – ARP is also seeking to expand its educational remit and the successful candidates will be offered a clinical academic colleague appointment with the relevant academic department.

Qualifications:

The successful candidate will be either accredited in General (Internal) Medicine and Geriatric Medicine or Family Medicine with a Care of the Elderly certification (or MD with equivalent qualification) and eligible for registration with the College of Physicians and Surgeons of Alberta.
