The Adult Community Health Navigators (CHNs) support WestView PCN patients with chronic diseases such as diabetes, kidney disease, heart disease, and lung disease, and improve their access to health care and other resources available in their community. Our CHNs help patients find health, social, and cultural resources in their communities. The CHNs work with patients, communities, physicians, and other healthcare personnel to provide the best care possible.
CHNs can meet with patients virtually, in person, or as a combination of both to support patients living with chronic diseases. Our CHNs are trusted members of the primary care network who:
• have strong roots in the community they serve and are experts in available health, social, and cultural resources, and services,
• are trusted members of the primary care team who can accompany patients through their healthcare journey, and
• have extensive on-the-job training in health system navigation and social support and receive support from the professional team (but do not have professional clinical education).
CHNs are mobile and work with patients in the home, in our clinics, and out in the community. They accept direct referrals from PCN physicians and clinical associates.
