At the WestView PCN, we aim to strengthen the care our community members receive for their health needs and their relationships with their family doctors. A patient attached to a family doctor has a base in the health care system called a medical home where their health needs are managed and coordinated at every stage of life.

At your medical home, your family doctor and health team care for you in collaboration with other service providers and facilities in your health neighbourhood at no out-of-pocket cost. Your family doctor’s medical practice is where you should feel comfortable discussing your health needs with a dedicated team that knows you and your medical history best.
