Primary Care Networks (PCNs) are Alberta's creative approach to comprehensive and interdisciplinary health care. PCNs are created through an agreement between physicians and Alberta Health Services. A PCN follows a team-based healthcare model where doctors and healthcare professionals work collaboratively to provide integrated care for all of the patients' primary healthcare needs.
Within a PCN, a family doctor's clinic is the patient's base in the health care system and is called a Patient's Medical Home (PMH) or Health Home. PMHs are the future of family medicine in Canada. Becoming a PMH means the family physician and team are committed to changing the way the care is delivered in pursuit of continual improvement supported by appropriate funding and infrastructure.
The PMH can be broken down into three themes: Foundations, Functions, and Ongoing Development.
