The Social Worker provides support, advocacy, and the coordination of services for clients with complex needs. They offer support and help in directing you to social and community services you might need, and navigate through the system and get different kinds of assistance such as mental health referrals, family support, financial crisis, seniors’ services, counseling referrals, and more.
The WestView PCN Social Worker co-locates between the WestView PCN office clinical space and Symphony Medical Clinic. The Social Worker only accepts direct referrals at both locations from PCN physicians and clinical associates via WestView PCN Clinical Fax (780) 960-9591.
If you do not have a doctor, visit our family doctors page to find one, and remember to check out the patient education classes. Our PCN has a Virtual Care Clinic too.
Referral Criteria
Must be 18 and over.
